Lessons from the field: Integrating behavioral and physical health care

A new study published last month in the Journal of the American Board of Family Medicine discusses efforts to address behavioral and physical health collaboratively among 11 practices participating in the Advancing Care Together program, a four-year initiative funded by The Colorado Health Foundation to test evidence-based methods for integrating care in real-world settings.

Both primary care practices and behavioral health providers were included in the study.

Workflow, leadership, culture change and adequate data management were among the challenges faced by the integration teams. Among the authors’ recommendations are identifying and enhancing methods for tracking and using data for quality improvement initiatives.
